Going to hospital can be very stressful!
You might be worried and scared.
Your parent or carer may be worried too.
In hospital you will get asked a lot of questions.
Different people in hospital might ask the same questions.
This can be frustrating!
When in hospital you need to be looked after.
The A2D Together Folder tells people how to look after you.
The folder has important information about you.
It will help hospital staff to look after you.
A folder can help you be less worried in hospital.
The folder will have your medical information.
It will have your address.
It can have your parent or carer’s name and phone number.
It will say what can make you sick.
It will tell people what can make you upset.
It will tell people how best to talk with you.
It will tell people what you do and don’t like.
It will tell people how you like to be helped.

This is how to make an A2D Together folder
There are 5 steps to make the folder
Type information into the white box. Type your name and what you like people to call you.
If you have a guardian or person responsible include their details. Type their name and phone number in the box under “If person is unable to consent to treatment please call:”
Add a recent photo of yourself into the “Insert photo here” box.
Step 2: The medication section
This section has information on your medications.
You will need your blister pack or your current medication chart.
If you have a blister pack put it in this section before you go to hospital. If you have a medication chart put it in this section before you go to hospital.
You will need to bring all your medications to hospital.
Keep medications in their packages and boxes.

Step 3: The TOP 5 section
This section has important things about you.
This is called your TOP 5.
The TOP 5 will tell hospital staff 5 important things about you.
Your TOP 5 tells people:
- how you communicate
- what you need help with
Your TOP 5 tells people:
- what you like
- what you don’t like
It tells people how you want to be helped.
Type your 5 important things into the boxes next to the numbers.
Print 3 copies of your TOP 5 page.

Step 4: The Support Plans section
The Green Section has your support plans
You might have a support or management plan for:
- eating and drinking
- to help with behaviour
- Epilepsy management
- Diabetes management
- Bowel management
- Pressure care management
